All Aboard

Thank you for choosing Life Unleashed as your pet transport. Prior to departure, we ask that each pet have his or her parent take a moment to go over this checklist. Reading over this information will enable you to have a most enjoyable road trip.

Pet Parent Checklist:

(1) HEALTH- An Interstate Health Certificate is a form distributed by the state of origin Department of Agriculture and completed, dated and signed by a licensed Veterinarian. A computer-generated form is not considered an Interstate Health Certificate. This is a requirement of the USDA. Also be sure that if you are sending a dog, that he/she has had a Bordetella (kennel cough) shot as well.

(2) FOOD- If you are sending food with your pet(s), please have only enough food for the transport in one or two zip-lock bags for each pet, depending on the size of your pet(s). Please also include specific, written feeding instructions for your pet(s).

(3) TOYS & BLANKET Please pack a favorite toy or blanket per pet (no chew toys).

(4) COLLARS - Please do not use a quick-release collar as these have a tendency to break. The collar you use should fit your pet well but not be too tight or too loose. You MUST attach accurate id with your new location information as well.

(5) EXTRAS- You may send a bowl or extra leash or collars with your pet(s). At pick up, we will store the items in your pet's assigned suitcase.

(6) SLEEP OVER- Please have a pillow-type bed ready to go with your pet or arrange for us to purchase one through our company. If we are purchasing, please allow at least 24 hrs prior to our buses leaving their points of origin to ensure the bed is ready for the trip.

(7) MEDICATION- If your pet is on medication, please send only the necessary medication needed for the transport. We also need to be informed if refrigeration is required in advance. If medication or shots need to be provided by a vet we will have to arrange for that prior to departure. Please provide detailed information about the administration of medication for each pet. We also require that you provide the contact information of your vet.

(8) ONLINE DAILY TAILS JOURNAL- Each pet will have his own journal where he can update details about his trip for his parent to read.

(9) PET SAFE TRACKER-Prior to departure, each pet will be assigned a tracking number so parents can log-in online and track the location throughout the transport.

(10) ADDITIONAL INFO-Please provide a detailed list of any habits, personality traits, likes/dislikes and anything else you feel we should have about your pet for the trip. This will help to ensure your pets receive what they need to make the trip as comfortable and enjoyable for them as possible.

(11) EMERGENCY VET FORM-We require that you fill out a consent form that grants us permission to take your pet to an emergency clinic if ever necessary.

(12) CONTACT INFO-Life Unleashed must be able to contact responsible parties at the scheduled departure and arrival times and places. For this reason, we require 2 contact names, phone numbers, and addresses for both the sender and receiver (see transport agreement).

(13) FORMS-Please complete and sign the Pet Shipping Agreement form and email it or fax it to us at (303) 302-1340. We must receive this information before we can pick up/transport your pet(s).
